Complete method of making Hummat Card

What is Hummat Card?*
Hummat Card is a program of the Government of Punjab which has been launched to provide financial assistance and facilities to the disabled (Special Persons).

✅ Eligibility (Who can get it?)

You are eligible for Himmat Card if:

You are a resident of Punjab province

You have a Computerized National Identity Card (CNIC) or B Form

A disability certificate from a government hospital or social welfare department

You are not benefiting from any other government financial assistance (like BISP, Zakat, Bait-ul-Mal etc.)

Your PMT score is less than 45

And your status on the disability certificate must be not fit to work

📄 Required Documents

The following documents should be accompanied while applying:

Identity Card / B Form

Disability Certificate

Passport Size Photograph

Residence Proof (Electricity/Gas Bill etc.)

Medical Report (if any)

How to get Himmat Card

🔹 Online Method

1. Visit the official website:
   dpmis.punjab.gov.pk
2. Through CNIC Register
3. Enter your complete information in the form
4. Upload all documents
5. Submit the form
6. After verification, you will get a notification through SMS

🔹 Offline (by going to the office)

1. Go to your nearest Social Welfare Office or DHQ Hospital
2. Take your CNIC and Disability Certificate with you
3. Fill the form and get medical verification done
4. After approval, you will get the Himmat Card

ہمّت کارڈ کیا ہے؟*
ہمّت کارڈ حکومتِ پنجاب کا پروگرام ہے جو معذور افراد (Special Persons) کو مالی مدد اور سہولیات دینے کے لیے جاری کیا گیا ہے۔

✅ اہلیت (کون بنوا سکتا ہے؟)

آپ ہمّت کارڈ کے لیے اہل ہیں اگر:

آپ صوبہ پنجاب کے رہائشی ہوں

آپ کے پاس کمپیوٹرائزڈ شناختی کارڈ (CNIC) یا ب فارم ہو

سرکاری اسپتال یا سوشل ویلفیئر ڈیپارٹمنٹ کا معذوری سرٹیفکیٹ ہو

آپ کسی اور حکومتی مالی امداد (جیسے BISP، زکوٰۃ، بیت المال وغیرہ) سے فائدہ نہ لے رہے ہوں

آپ کا PMT اسکور 45 سے کم ہو

اور ڈس ایبلٹی سرٹیفکیٹ پہ آپ کا سٹیٹس not fit to work ہونا ضروری ہے

معذوری سرٹیفکیٹ

پاسپورٹ سائز تصویر

رہائش کا ثبوت (بجلی/گیس کا بل وغیرہ)

میڈیکل رپورٹ (اگر ہو)

🔹 آن لائن طریقہ

1. سرکاری ویب سائٹ پر جائیں:
   dpmis.punjab.gov.pk
2. CNIC کے ذریعے رجسٹریشن کریں
3. فارم میں اپنی مکمل معلومات درج کریں
4. تمام دستاویزات اپ لوڈ کریں
5. فارم جمع کروائیں
6. تصدیق کے بعد آپ کو SMS کے ذریعے اطلاع ملے گی 🔹 آف لائن (دفتر جا کر)
7. اپنے قریبی سوشل ویلفیئر آفس یا ڈی ایچ کیو اسپتال جائیں
8. شناختی کارڈ اور معذوری سرٹیفکیٹ ساتھ لے جائیں
9. فارم پُر کریں اور میڈیکل ویریفیکیشن کروائیں
